Thyroglobulin — What Your Blood Test Result Means

ScanHealth Learn Thyroid Thyroglobulin

Thyroid Tissue Marker

Thyroglobulin is produced exclusively by thyroid cells—it's the raw protein used to build thyroid hormones. Its main clinical use is monitoring after thyroid cancer treatment. If the thyroid has been surgically removed, thyroglobulin should be undetectable. If it shows up, thyroid tissue remains.

What is Thyroglobulin?

Thyroglobulin (Tg) is a glycoprotein produced only by thyroid follicular cells. Primary role is as a tumor marker after thyroid cancer treatment. Anti-thyroglobulin antibodies can interfere with measurement.

What High Thyroglobulin Means

In intact thyroid: can be from goiter, thyroiditis, or hyperthyroidism. Post-thyroidectomy: may indicate cancer recurrence or residual tissue.

Common symptoms:

May be asymptomatic · Neck swelling if goiter

What Low Thyroglobulin Means

Post-thyroidectomy: this is the goal. In intact thyroid: may indicate thyroid suppression.

Common symptoms:

Not typically symptomatic

Why It Matters

When normal:

Gold standard for thyroid cancer surveillance after thyroidectomy

Detects recurrence early

Monitors treatment response

Risks if abnormal:

Rising post-thyroidectomy: possible cancer recurrence

Antibody interference can mask results

What Can Cause Abnormal Levels?

Thyroid Cancer Recurrence (post-surgery)

40% likely

Detectable thyroglobulin after thyroid removal suggests remaining thyroid tissue or cancer.

Goiter or Thyroiditis

50% likely

Inflammation or enlargement increases thyroglobulin release from intact thyroid.

Graves' Disease

Thyroid stimulation increases production.

Iodine Deficiency

Iodine deficiency goiter increases thyroglobulin.

What You Can Do

Adequate iodine intake if deficient

Impact: Reduces compensatory elevation \u00B7 Timeline: 4-8 weeks

If lifestyle changes aren't enough:

Treat underlying thyroiditis or Graves' disease

Impact: Normalizes thyroglobulin \u00B7 Timeline: Varies

Recommended retest: 6-12 months for cancer surveillance
