How Full Are Your Iron Trucks?
Transferrin saturation tells you what percentage of your iron transport proteins are actually loaded with iron. Below 20% means they're running empty (iron deficiency). Above 45% means overloaded (possible hemochromatosis). It's more informative than serum iron alone because it factors in capacity.
What is Transferrin Saturation?
Transferrin saturation = (Serum Iron / TIBC) × 100. It's the percentage of iron-binding sites occupied. More reliable than serum iron alone. Normal: 20-45%.
↑ What High Transferrin Saturation Means
Iron transport is overloaded. Consistently above 45% is the best screening test for hereditary hemochromatosis—warrants genetic testing.
Common symptoms:
Often asymptomatic early · Joint pain (especially knuckles) · Fatigue · Bronze skin · Liver problems
↓ What Low Transferrin Saturation Means
Transport running empty—not enough iron. Classic iron deficiency, especially with low ferritin.
Common symptoms:
Fatigue, pallor, cold extremities, restless legs
Why It Matters
When normal:
Best single screening test for hemochromatosis
More informative than serum iron alone
Helps distinguish iron deficiency from inflammation
Risks if abnormal:
<20%: iron deficiency
>45%: hemochromatosis screening trigger
>60%: high risk of organ iron deposition
What Can Cause Abnormal Levels?
Iron Deficiency (low)
55% likelyLow iron + high TIBC = low saturation.
Hemochromatosis (high)
45% likelyGenetic excess iron absorption keeps transferrin heavily loaded.
Chronic Inflammation (low)
Iron trapped in cells reduces circulating iron and saturation.
Liver Disease (high)
Liver damage releases stored iron.
What You Can Do
If low: iron-rich diet with vitamin C
Impact: Increases circulating iron and saturation · Timeline: 4-8 weeks
If high: stop unnecessary iron supplements
Impact: Prevents further overload · Timeline: Immediate
If lifestyle changes aren't enough:
If consistently >45%: HFE gene test for hemochromatosis
Impact: Definitive genetic diagnosis · Timeline: One-time
If low: ferrous sulfate 325mg every other day
Impact: Restores iron and normalizes saturation · Timeline: 8-12 weeks
Recommended retest: 6-8 weeks if treating; 3-6 months routine
