Urine Bilirubin — What Your Blood Test Result Means

ScanHealth Learn Urinalysis Urine Bilirubin

Liver and Bile Duct Problem Detector

Bilirubin in urine is always abnormal and suggests a problem with the liver or bile ducts. Only conjugated (direct) bilirubin is water-soluble enough to appear in urine. This means the liver has processed the bilirubin but it's backing up—typically from a blocked bile duct or liver disease. The urine turns dark brown, often before jaundice becomes visible.

What is Urine Bilirubin?

Urine bilirubin dipstick detects conjugated (direct) bilirubin only. Unconjugated bilirubin is albumin-bound and not filtered. Positive urine bilirubin = conjugated hyperbilirubinemia (hepatocellular or obstructive cause). Absent in hemolytic jaundice (unconjugated).

What High Urine Bilirubin Means

Conjugated bilirubin is backing up into blood and spilling into urine. Causes: bile duct obstruction (gallstones, tumors), hepatitis, cirrhosis, drug-induced liver injury. Dark brown urine + positive bilirubin = obstructive or hepatocellular jaundice.

Common symptoms:

Dark brown/cola-colored urine (earliest sign) · Jaundice (yellow skin and eyes) · Pale/clay-colored stools · Itching (bile salt deposition) · If obstruction: right upper quadrant pain

What Low Urine Bilirubin Means

N/A.

Common symptoms:

N/A

Why It Matters

When normal:

Distinguishes conjugated from unconjugated jaundice

Positive = hepatocellular or obstructive cause

Negative in hemolytic jaundice (helpful distinction)

Dark urine may be first sign of liver disease

Risks if abnormal:

Always abnormal when positive

Suggests bile duct obstruction or significant liver disease

What Can Cause Abnormal Levels?

Bile Duct Obstruction

40% likely

Gallstones or tumors blocking common bile duct. Conjugated bilirubin backs up into blood and spills into urine.

Hepatitis/Liver Disease

35% likely

Hepatocellular damage impairs bilirubin excretion into bile.

Drug-Induced Liver Injury

Medications causing hepatocellular or cholestatic liver injury.

Pancreatic Head Tumor

Compresses common bile duct causing obstructive jaundice.

What You Can Do

If positive: check serum bilirubin (total and direct), liver enzymes

Impact: Characterizes liver/biliary disease \u00B7 Timeline: Immediately

If lifestyle changes aren't enough:

Right upper quadrant ultrasound to evaluate bile ducts

Impact: Identifies dilated ducts (obstruction) vs normal ducts (hepatocellular) \u00B7 Timeline: Urgent

Recommended retest: Resolves with treatment of underlying cause; not independently monitored
