Bilirubin in urine is always abnormal and suggests a problem with the liver or bile ducts. Only conjugated (direct) bilirubin is water-soluble enough to appear in urine. This means the liver has processed the bilirubin but it's backing up—typically from a blocked bile duct or liver disease. The urine turns dark brown, often before jaundice becomes visible.
What is Urine Bilirubin?
Urine bilirubin dipstick detects conjugated (direct) bilirubin only. Unconjugated bilirubin is albumin-bound and not filtered. Positive urine bilirubin = conjugated hyperbilirubinemia (hepatocellular or obstructive cause). Absent in hemolytic jaundice (unconjugated).

Conjugated bilirubin is backing up into blood and spilling into urine. Causes: bile duct obstruction (gallstones, tumors), hepatitis, cirrhosis, drug-induced liver injury. Dark brown urine + positive bilirubin = obstructive or hepatocellular jaundice.
