Vitamin B7 (Biotin) — What Your Blood Test Result Means

ScanHealth Learn Vitamins Vitamin B7 (Biotin)

The Hair, Skin, and Nails Vitamin (With a Lab Warning)

Biotin is essential for carboxylase enzymes involved in gluconeogenesis, fatty acid synthesis, and amino acid metabolism. It's widely marketed for hair, skin, and nails—though evidence for supplementation in non-deficient people is limited. CRITICAL LAB WARNING: biotin supplements interfere with many blood tests (thyroid, troponin, hormones) and can cause dangerously misleading results.

What is Vitamin B7 (Biotin)?

Biotin (vitamin H) is a cofactor for carboxylase enzymes (pyruvate carboxylase, acetyl-CoA carboxylase, propionyl-CoA carboxylase). RDA: 30μg/day. Many supplements contain 5-10mg (150-330x RDA). Biotin is used in streptavidin-biotin immunoassay platforms—high-dose supplementation interferes with these assays.

What High Vitamin B7 (Biotin) Means

Supplemental biotin at high doses (5-10mg) can cause FALSE lab results: falsely high thyroid hormones (looks like hyperthyroidism), falsely low TSH, falsely low troponin (could miss heart attack). Stop biotin 48-72h before any blood work.

Common symptoms:

No toxicity from biotin itself · DANGER: false lab results if blood drawn while supplementing

What Low Vitamin B7 (Biotin) Means

Rare. Causes: raw egg white consumption (avidin binds biotin), pregnancy, anticonvulsants (carbamazepine, phenytoin), genetic biotinidase deficiency.

Common symptoms:

Hair thinning/loss · Scaly red rash (perioral, periocular) · Brittle nails · Depression, fatigue · Biotinidase deficiency: seizures, hearing loss, developmental delay

Why It Matters

When normal:

Essential for gluconeogenesis and fatty acid metabolism

Supports hair, skin, and nail health (though evidence for supplements is limited)

Biotinidase deficiency screening is part of newborn screening

Risks if abnormal:

MAJOR: high-dose supplements interfere with lab tests (thyroid, troponin, hormones)

Can cause false hyperthyroidism diagnosis

Can mask troponin elevation (miss heart attack)

Must stop 48-72h before blood work

What Can Cause Abnormal Levels?

Raw Egg Whites (deficiency)

40% likely

Avidin in raw egg whites binds biotin and prevents absorption. Cooking denatures avidin.

Biotinidase Deficiency (genetic)

20% likely

Autosomal recessive. Can't recycle biotin. Newborn screening catches it.

Pregnancy

Marginal biotin deficiency is common in pregnancy.

Anticonvulsants

Carbamazepine, phenytoin, and phenobarbital increase biotin catabolism.

What You Can Do

CRITICAL: Stop biotin supplements 48-72h before ANY blood work

Impact: Prevents false thyroid results, false troponin, false hormone levels · Timeline: 48-72h before labs

Dietary sources: eggs (cooked), nuts, seeds, salmon, sweet potato

Impact: Diet provides adequate biotin for most people · Timeline: Ongoing

If lifestyle changes aren't enough:

If supplementing for hair/nails: 2.5-5mg daily (evidence is limited)

Impact: May help brittle nails. Hair evidence is weak in non-deficient individuals. · Timeline: 3-6 months trial

Recommended retest: Rarely tested directly; tell your doctor about biotin supplements before any lab work
