Urobilinogen is a byproduct of bilirubin metabolism in the gut. Normally, a small amount is reabsorbed and excreted in urine (0.2-1.0 mg/dL). When it's elevated, either too much bilirubin is being produced (hemolysis) or the liver can't recapture it (liver disease). When it's absent, bile isn't reaching the gut (complete obstruction).

What is Urine Urobilinogen?

Urobilinogen is produced by gut bacteria from conjugated bilirubin. ~80% is excreted in stool (as stercobilin—gives stool brown color). ~20% is reabsorbed (enterohepatic circulation) and either recaptured by liver or excreted by kidneys in urine.

What High Urine Urobilinogen Means

Increased bilirubin production (hemolytic anemia) or impaired hepatic uptake (hepatitis, cirrhosis).

Common symptoms:

If hemolysis: fatigue, pallor, jaundice · If liver disease: jaundice, fatigue, abdominal discomfort

What Low Urine Urobilinogen Means

Complete biliary obstruction (no bilirubin reaching gut → no urobilinogen produced) or broad-spectrum antibiotics (kill gut bacteria that produce urobilinogen).

Common symptoms:

If obstructive: pale stools, dark urine, jaundice, itching

Why It Matters

When normal:

Absent urobilinogen + positive urine bilirubin = obstructive jaundice

Elevated urobilinogen = hemolysis or hepatocellular disease

What Can Cause Abnormal Levels?

Hemolytic Anemia (high)

Complete Biliary Obstruction (absent)

No bilirubin reaches gut → no urobilinogen produced → absent from urine AND stool turns pale.

Broad-Spectrum Antibiotics (low)

Kill gut bacteria that convert bilirubin to urobilinogen.
